You can use the code below to add the Rare email signup form to any site. You can then modify any of the CSS classes to make it look how you want:

The bolded text is your store's SUBMIT URL, which is different for every store (this is an example here).

You can find your store's HTML code and Submit URL in your Rare account under your Account Name>Contacts and selecting the 'Signup Form' tab. 

You can create different lists with different signup forms as well. When you go to create a new list (Lists>New) by selecting the 'Signup Form' tab, you can have access to this specific list's HTML code and Submit URL.
